Contract | 13-Week Assignment | Full-Time | Days | Monday – Friday, 40 hours per week | Onsite

Key Responsibilities

• Provide occupational therapy services to patients in a home health setting within an assigned territory
• Conduct evaluations and develop individualized treatment plans to address patients' functional needs
• Deliver skilled occupational therapy interventions aimed at improving patients' independence and quality of life
• Document patient progress, treatment plans, and clinical notes in accordance with home health standards
• Coordinate care with interdisciplinary team members to ensure comprehensive patient outcomes

Required Qualifications

• Active and valid Occupational Therapist (OT) license in the state of Wisconsin
• Minimum of 12 months of home health experience within the past 3 years
• Strong clinical documentation and communication skills
• Ability to work independently in a home health environment

Preferred Experience / Skills

• Experience with HCHB (Homecare Homebase) electronic medical records system
• Familiarity with home health regulatory and compliance requirements
