\nJob Duties
\n \nYou'll primarily work office based with some requirements for on-site work. You'll coordinate patient discharges from the hospital to home infusion settings and support ongoing patient care in both home and infusion suite environments.
\n \nAt least three (3) years of RN work experience is required.
Case management experience, RN Home Health and IV therapy experience is preferred.
A typical schedule is part time, 20 hours weekly, 8:30 am to 5 pm, with rotating weekends, holidays, and some call

Position Details
\n \nHome Infusion Nurse is responsible for coordinating and delivering comprehensive infusion therapy services in both home, suite, and other clinical settings. This role involves managing referrals, verifying clinical and administrative requirements, and providing direct patient care, education, and case management. The nurse collaborates with physicians, pharmacists, and interdisciplinary teams to ensure safe, effective, and patient-centered care throughout the continuum.
