The Associate Medical Director at CenterWell Home Health plays a key clinical leadership role in advancing high-quality, in-home healthcare services for a diverse adult population. Working within the Healthcare & Medical Services department, this physician leader helps ensure that patients receive safe, evidence-based care that promotes autonomy, functional independence, and quality of life in the comfort of their homes. In close collaboration with the Medical Director and operational leaders, the Associate Medical Director provides clinical oversight for home health programs, including skilled nursing and rehabilitative services. This role helps shape and refine clinical protocols, reviews complex and high-risk cases, and ensures that individualized care plans are appropriate, safe, and aligned with patient goals. The Associate Medical Director actively participates in interdisciplinary case conferences, supporting nurses, therapists, and other clinicians in delivering coordinated, patient-centered care. The Associate Medical Director is responsible for monitoring clinical quality, patient safety, and regulatory compliance across assigned teams or regions. Using clinical data, quality indicators, and outcome measures, this role identifies trends, recommends improvement strategies, and supports the implementation of quality improvement initiatives. The Associate Medical Director helps maintain compliance with Medicare (CMS) regulations, state requirements, and relevant accrediting standards, reinforcing CenterWell’s commitment to integrity, accountability, and excellence. A key aspect of the position is providing clinical guidance, education, and mentorship to field clinicians. The Associate Medical Director offers consultation on complex diagnoses, medication management, chronic disease management, and appropriate utilization of services in the home setting. By fostering open communication and collaboration, this role supports a culture where clinicians feel empowered to raise concerns, discuss clinical challenges, and continuously enhance their practice. The Associate Medical Director also contributes to strategic initiatives aimed at improving patient outcomes and operational efficiency. This may include helping to develop new care pathways, integrating evidence-based guidelines into home health practice, and supporting innovative approaches to telehealth and remote monitoring where appropriate.
